Application Form

Complete the application and submit $60.00. Check or money order can be sent or dropped off at the address to the right. Checks should be made out to Phi Eta Sigma National Honor Society at UCF. The membership fee is only required at the time of application. There are no fees collected after inducted into Phi Eta Sigma.

NOTE: Students submitting an application will be inducted during the Fall 2012 ceremony.
